The second phase of our intervention involves creating a plan to prevent future crises. This includes evaluating the root causes of the crisis, adjusting care plans, and coordinating with healthcare providers and other professionals to implement long-term solutions. Aging with Grace Care Management is available to provide ongoing care management support to monitor the plan of care and make necessary adjustments to avoid future crises
