Understanding the urgency, our first step is to assess the situation quickly and efficiently, identifying the needs and risks involved. We then mobilize our resources to provide immediate assistance, whether it involves medical, emotional, or environmental crises. Our trained team is adept at handling unexpected challenges, ensuring the safety and well-being of the senior while minimizing stress and anxiety for the family.

The second phase of our intervention involves creating a plan to prevent future crises. This includes evaluating the root causes of the crisis, adjusting care plans, and coordinating with healthcare providers and other professionals to implement long-term solutions. Aging with Grace Care Management is available to provide ongoing care management support to monitor the plan of care and make necessary adjustments to avoid future crises

Upon receiving a call for crisis intervention our team responds promptly. We assess the situation, provide immediate support, and work to stabilize the crisis. Following the immediate response, we collaborate with relevant professionals to develop a comprehensive plan that addresses the immediate needs and outlines steps to prevent similar situations in the future.

I met Rosanne in 2019 when she was serving as the geriatric care manager for a husband and wife who were at a very difficult stage of their lives and who had no local family to help with the daily medical needs they were requiring. Roseanne reached out to me, as she saw the need for a primary care physician who could provide home visits to this aging couple. Without the extensive services that Roseanne put in place, this couple would have surely been separated, one in a memory care facility and the other likely in a nursing home. They continued to live at home together with the expert help provided by Roseanne. I continue to work with Roseanne and trust that she will provide the most compassionate care and recommendations to my elderly patients.
