Rose Anne Dioguardi,
MSW, LICSW, C-ASWCM
Rose Anne has over 20 years experience working in the fields of elder care and healthcare as a medical social worker and certified care manager. Her professional journey began as a social worker in skilled nursing facilities and then transitioned to doing inpatient social work/case management at the Wyman Center at Mount Auburn Hospital working in the field of geriatric psychiatry. This laid a foundation for the beginning of her career as a Geriatric Care Manager at Massachusetts General Hospital, where she was able to develop her skills and strong knowledge base as a care manager. She is a tenacious advocate for her clients and their families. Her goal is to preserve as much autonomy as possible for her clients while also ensuring that they receive the utmost care. She has an extensive understanding of the healthcare system, community resources and insurances. As a medical social worker she has gained tremendous knowledge of many of the illnesses-both medical and psychiatric that her clients face and is comfortable working in the medical arena serving as a voice for her clients. Rose Anne earned her Masters in Social Work in 2001 from SUNY Albany School of Social Welfare. She is a member of the National Association of Social Workers and is an Advanced Professional of the Aging Life Care Association. Outside of her work life, Rose Anne enjoys spending time with her husband Patrick and their dog, Charlie. She loves to spend time in her garden, go to the beach and practice yoga. She attributes her love for seniors to her beloved grandmothers for whom she is named after, who taught her first-hand to honor the person within no matter their age using compassion, empathy and grace.

With nearly 20 years of experience as a licensed Occupational Therapist, she brings a sophisticated, client-centered approach to geriatric care management. She holds a degree in Occupational Therapy from the University of New Hampshire, grounding her work in both clinical rigor and a deep understanding of functional independence across the lifespan.
She began her career with Cambridge Health Alliance, where she developed a strong clinical foundation across the full continuum of care—from inpatient medical/surgical units to outpatient hand therapy. This early breadth of experience shaped her holistic perspective and ability to assess and address complex, evolving needs.
Her career has since expanded to include skilled nursing, where she worked as a traveling therapist across diverse facilities, as well as home health care with Comfort Home Care, where she was responsible for managing clients throughout the North Shore. These roles further strengthened her adaptability, resourcefulness, and nuanced understanding of care delivery in varied settings.
She is recognized for her ability to establish trust quickly and navigate complex situations with clarity, discretion, and compassion. Combining strong clinical expertise with emotional intelligence, she develops highly individualized care plans that are both practical and responsive to each client’s goals and circumstances. Her work is grounded in thoughtful collaboration with families and care teams, ensuring a seamless and supportive experience.
Through her role as a remote care planner, she has further refined her expertise in care coordination and proactive planning, guiding families through the evolving challenges of aging with insight and reassurance. She is particularly skilled at bringing structure and clarity to complex care needs, allowing clients and their families to move forward with confidence. She is committed to delivering discreet, detail-oriented support that upholds each client’s dignity, independence, and quality of life.
